I think it was only previously mentioned in The Phantom Menace, and skimmed over there. Mace Windu says "You refer to the prophecy of The One, who will bring balance to the Force. You believe it's this boy?", but I can't think of a single other reference.

Having said that, I believe that Anakin (rather than Luke) is the Chosen One. In RotS, Mace Windu, Yoda and Obi-Wan all state definatively that the prophecy *does* refer to Anakin, but agree that it might have been misinterpreted.

Anakin is trained as a Jedi, becomes a Sith, and returns to the light side when he dies; he is balanced between both sides of the force, and is directly responsible for the tearing down of the old orders of both Jedi and Sith.

Luke cannot bring balance to the force as he has no balance; he is pure Jedi. His story is only a small part in the filling of Anakin's destiny.
