I've always used my middle name (My parents didn't like the thoguht of me having the initials RAT, so they swapped my names around).

The only problem I've ever had is trying to remember which name I'm registered under at the doctors, or somewhere. That normally takes a little while to clear up, but there are probably branches of officialdom where they have my listed under two different names.
