John,
Your bike was probably made in Austria or Italy. Possibly by Puch. I'm not at home right now, but when I get back, I'll try to look it up.

I know a guy who collects nothing but bikes sold by Sears. He might be interested it the manual. How much do you want for it? And if you still have the bike I would be willing to drive to Ohio if I could get a good deal.
