If I recall correctly, the enrollment sheet should be in the administration building. If it's not there, then it would be either the bookstore or the building where the English prof has an office.

I can't narrow it down any more than that, as I haven't played TOA in a while. But I'm reasonably certain the item you seek is around where I said it should be.
