Honestly, simply the perceptions of the voters. Oscar history is littered with circumstances where this strange distinction is made in confusing ways. Louise Fletcher could hardly be called a main character of "One Flew Over the Cuckoo's Nest." With little screentime, still she won a Best Actress Oscar.

Both Frances McDormand and William H. Macy were nominated for "Fargo" -- McDormand, for Best Actress; Macy, for Best Supporting Actor. I actually agree with these decisions, because McDormand so dominates one's memory of that film. Still, McDormand had just less than 30 minutes of screentime in it, whereas Macy had just a bit more than 30 minutes of screentime.
