I have seen a new practice lately--one which acknowledges the problem without being insane about it. Some works use either "he" or "she" but in a continuous context where the individual referred to is clearly the same, the same pronoun is used... Overall, though, the two are used about evenly. It struck me as a bit odd at first, but I don't mind it now, and I would hope that it would be a satisfactory solution for those who care about such things.
