I would be willing to bet that the Al Qa'ada (or something like that) don't have a plastic surgeon on their books. There can't be much demand for it in Afganistan, considering that not one legally practicioning doctor is qualified to do it. And if I was bin Laden, I would have difficulty trusting that a doctor who wasn't a member of his organisation wouldn't put you unconcious and then claim the $5,000,000 reward.

Even so, I think that cosmetic surgery is a great sin in the eyes of Islamists.

I could, of course, be wrong on either of the above points.

His face is probably one of the most famous in the world at the moment, and given that he won't cut his beard (for religous reasons), he would have great difficulty going anywhere by air (and the local terrain is not condusive to rapid ground travel).

Besides, while the Taliban may not be willing to surrender bin Laden, they may well not be averse to proving that he is no longer in Afghanistan if it would stop Kabul being carpet bombed.
