Because there's no advertising. People have to pay to watch it. And interest groups seem to have a hard time convincing people not to watch programs, but they're good at getting advertisers to pull their money away. They can't organize a boycott of some advertisers product on HBO because their aren't any. They could try to boycott HBO itself, but people are already paying that money for a reason - TV MA television (oh, and maybe movies and boxing, but I doubt it.) So you can't get people to boycott something that is the reason they watch it in the first place.
