Two things not to forget:

1) Many addictions, particularly something as physically consuming as this, are not an enemy to be conquered and reign victorious over. It's a daily battle, even if the temptation does get easier.

2) The poem is from the addict's perspective, apparently not by one who has rehab'ed, so the "forever" sentiment is from that blinded perspective, not from one with hope of escape.
