Train more. If you're good at hill starts, then you got the main question in using a manual transmission; the rest is only a matter of learning some easy techniques.

For example: first gear and reverse gear go in easy only if the car is stopped (well, if the car is moving, you can always use brute force to get them in).
Parking is another set of easy techniques.

It's an issue that affects many recent cars, due to their "wedge" design; you have many options:
- borrow a car that has a better rear view than yours just to pass the exam;
- ask someone to help you in training;

I had a similar problem years ago, when I had to drive my father's van; it's a small FIAT van with no rear view at all (the rear door has no glasses); my solution: I went to a place in the suburbs, where I had all the space I needed; I used some trash cans to simulate a car parked behind me and trained to use the (small) side mirrors to help me in doing all the maneuvers - trying not to touch the "obstacles".
It's all a question of using the side mirrors and keeping in view some reference point near to the car to measure distances.
