I highly doubt that a stream of air could be focused to this intensity, or to the speeds needed to remove a filling. Probably he had one of the water-laser drills.

My dentist is quite good, actually-- best one I've ever had. I went in to get a filling, and I was out in twenty minutes. I didn't feel any pain, either. Oh, it was unpleasant as hell, but not painful.

BTW: did you know that the people who invented Novocain were trying to synsethize cocaine? Cocaine was used as an anesthetic in dental surgery.
