Hold his arms and encourage him that it is going to be ok. Plug his nose so that he has to open his mouth and as he opens his mouth (and after he breaths!), make him drink in very small doses so he doesn't choke. Give him big hugs for doing such a great job for not leaving you and him a big mess!

Additionally, the dropper allows you to place the medicine where you please (providing the little nipper is sufficiently restrained). Shoot the stuff right down the back of his throat. This makes it much harder for him to spit it back at you as well.
When you have the option of giving him a flavored medicine, do so. If they form the idea that medicine actually tastes good, they'll be far less argumentative when you have to slip in the icky stuff. Case in point, after watching my 1½ year old son take some children's Tylenol, my 4½ year old daughter said that she, too, didn't feel well and that she needed some medicine. She was disapointed that we didn't give her any.

On a somewhat related note, she has finally figured out that Bactine doesn't sting like Hellfire, and will actually allow herself to be sprayed. Up to this point, though.... I'll just say that she can be very determined when she wants to be.
