Maybe I misremembered it when I sent it in, but I think that either I didn't write it clearly, or that someone else who heard it from me sent it in wrong. I am pretty sure it was a 3.5" floppy disk.

I don't think it is someone else's anecdote, although that is possible, though a bit unlikely.
