Do I need to complete the VP tasks along with completing the other task throughout the game. Which then means I have to start over ??????? I also believe I need another item from the girls. The VP tasks are for accessing the secret area. They are not necessary to complete the game. You do not need the scalpel anymore for either.

HOWEVER, there is one item necessary to access the secret area that you do not have and can no longer achieve since you put the classful of brats to sleep already. So forget about the secret area, forget about the VP, you can't get there and it's not necessary to beat the game.

Having said that, have you taken a "leap of faith" that's landed you in the clinic?

If so, good, you got the guy away from the truck. I don't believe he locked the truck, and a costume change can be found in there. Fortunately, there are a couple places left you can still go change unseen.

If not, get back on the roof and go do something potentially suicidal that your character's wanted to do for ages.
