The random option will take you either north, south, east or west of the guard. West is the pit. South is the hall of doors and the tunnel that takes you to the pit. East is damp. I think North is the way you ran when you encountered the guard. If you get damp or the guard again, you'll be able to make your way to the rest of the dungeon (instead of the endless loop of doors, pit and tunnel).

I actually mapped this game out, but even the map is difficult to explain. So this is all I can tell you about your missing item: when you have the opportunity at a cell door, look left.
