Philip Glass has done this. He wrote soundtracks for two films that lacked them ("Dracula" and "Cenere") and re-wrote the soundtracks for three others ("La Belle et la Bête," "Orphée," and "Les Enfants Terribles"). See link below for details (Trivia Item 1).
